I understand that if I cancel my membership outside of the 30 day notice to my next billing date, I will be charged one final billing cycle. Any available credits in my account are non-refundable, but will not expire. Services after cancellation will be redeemable at VIP pricing for 30 days from the date of final payment made.

I understand that cancellations through this request do not cancel any upcoming appointments. Cancellations per my profile will be made on my behalf by calling 702-751-9331 or cancelling online in my FloatHelm profile.
